Testosterone is often discussed almost entirely in relation to sex drive, muscle building or gym performance. In reality, its role in men’s health is much broader.

The hormone contributes to sexual function, sperm production, muscle mass, bone strength and general physical wellbeing. When testosterone levels are genuinely too low, the effects can therefore extend well beyond the bedroom.

At the same time, testosterone is sometimes blamed for symptoms that may have completely different causes. Feeling tired, gaining weight or experiencing erectile difficulties does not automatically mean testosterone levels are low.

Understanding what testosterone actually does, how it changes with age and when symptoms deserve investigation can help separate genuine hormonal problems from common misconceptions.

What is testosterone?

Testosterone is the main male sex hormone and is produced primarily in the testes.

Its production is controlled through signals involving the brain and pituitary gland, meaning normal testosterone levels depend on several parts of the hormonal system working together.

Testosterone plays an important role during puberty, contributing to changes such as growth of facial and body hair, deepening of the voice and development of the reproductive organs.

Its importance does not disappear once puberty ends.

Throughout adult life, testosterone continues to contribute to sexual function, muscle and bone health and the production of sperm.

Testosterone and sexual health

One of testosterone’s best known roles is its effect on sexual desire.

Men with genuine testosterone deficiency may experience reduced libido and fewer spontaneous or morning erections. Severe deficiency can also contribute to erectile dysfunction.

However, erectile dysfunction should not automatically be interpreted as evidence of low testosterone.

Achieving an erection depends on healthy blood flow, nerves, psychological wellbeing and several other aspects of health. Persistent erectile difficulties can be associated with high blood pressure, raised cholesterol, diabetes, anxiety, depression, certain medicines and smoking as well as hormonal problems.

Erectile dysfunction treatments such as Viagra contain sildenafil, which increases blood flow to the penis in response to sexual stimulation.

It does not increase testosterone levels and it does not treat testosterone deficiency.

Sildenafil is also unsuitable for some people, particularly those taking nitrate medicines for conditions such as angina, because combining the two can cause a potentially dangerous fall in blood pressure.

Testosterone helps maintain muscle

Testosterone contributes to the maintenance of muscle mass and strength.

This is one reason severe testosterone deficiency can be associated with reductions in muscle mass, strength and exercise capacity.

However, testosterone is only one part of the picture.

Muscle mass is also strongly influenced by physical activity, resistance exercise, protein intake, overall nutrition, illness and age.

For most men, regular strength training remains one of the most important things they can do to maintain muscle throughout adulthood.

Weight training, resistance bands and challenging body weight exercises can all help. The objective does not have to be bodybuilding. Maintaining strength supports mobility, metabolic health and independence as men grow older.

Taking testosterone without a medically confirmed deficiency is not a substitute for exercise and appropriate nutrition.

Bone health depends on testosterone too

The relationship between testosterone and bone health receives considerably less attention than its effect on muscle.

Men with long term testosterone deficiency can experience a reduction in bone density, which may eventually increase the risk of osteoporosis and fractures.

Bone health becomes increasingly important with age because fractures, particularly involving the hip or spine, can have a major effect on mobility and independence.

Regular weight bearing exercise, strength training, adequate nutrition and sufficient calcium and vitamin D all contribute to maintaining healthy bones.

When genuine testosterone deficiency is identified, addressing the underlying hormonal problem may also form part of protecting long term bone health.

Testosterone is important for fertility

Testosterone is also closely involved in sperm production.

The testes require high local concentrations of testosterone to support the production of healthy sperm.

However, there is an important distinction between the testosterone the body produces naturally and taking testosterone from an external source.

Men who are considering testosterone replacement and who want to have children should discuss fertility with their doctor. Taking external testosterone can suppress the hormonal signals that normally stimulate the testes and can significantly reduce sperm production.

This is why testosterone treatment is not simply something that should be started because someone feels tired or wants to improve performance.

A proper diagnosis and discussion of the potential benefits and risks are important.

What happens to testosterone as men get older?

Testosterone levels generally decline gradually with age.

The NHS notes that levels typically fall by around 1% per year from approximately the age of 30 to 40 onwards.

This does not mean every man develops testosterone deficiency.

A gradual age related decline is very different from clinically significant hypogonadism, which occurs when the body is not producing enough testosterone and this is accompanied by relevant symptoms.

The idea of a sudden “male menopause” can therefore be misleading.

Unlike the relatively rapid hormonal transition experienced by women during menopause, testosterone usually declines gradually in men.

Many symptoms commonly blamed on declining testosterone, including tiredness, poor sleep, low mood, weight gain and reduced libido, may instead be related to stress, depression, anxiety, medication, poor sleep, alcohol, obesity or another health condition.

What are the signs of genuinely low testosterone?

Testosterone deficiency can produce a range of symptoms.

Possible signs include a persistent reduction in sex drive, erectile difficulties, reduced spontaneous erections, fatigue, reduced strength, loss of muscle, difficulty concentrating and changes in mood.

Long term deficiency can also affect bone density.

The difficulty is that none of these symptoms proves that testosterone is low.

Fatigue, for example, can result from inadequate sleep, anaemia, thyroid problems, depression, medication or many other causes. Erectile dysfunction may be primarily cardiovascular or psychological.

Diagnosis therefore involves more than a symptom questionnaire.

A healthcare professional will normally consider the symptoms alongside blood tests measuring testosterone. Depending on the results, additional investigations may be required to establish why levels are low.

Lifestyle still matters

Although lifestyle changes cannot correct every cause of testosterone deficiency, general health can influence hormone levels as well as many of the symptoms that people associate with low testosterone.

Regular exercise, maintaining a healthy weight, sleeping adequately, eating a balanced diet and limiting excessive alcohol consumption all support broader physical health.

Resistance training is particularly useful for maintaining muscle and strength, while adequate sleep deserves attention because consistently poor sleep can affect energy, mood and sexual wellbeing regardless of testosterone levels.

Men carrying excess weight may also benefit from gradual weight loss for broader metabolic and cardiovascular reasons.

The important point is not to treat testosterone in isolation.

Sexual function, energy and physical performance depend on cardiovascular health, mental wellbeing, sleep, nutrition, activity and hormones working together.

Avoid unregulated “testosterone boosters”

Supplements marketed as testosterone boosters are widely available online and through the fitness industry.

The fact that a product claims to support testosterone does not mean it will correct a genuine hormone deficiency.

If symptoms suggest low testosterone, blood testing and clinical assessment are considerably more useful than trying multiple supplements.

Men should also be cautious about obtaining testosterone or anabolic steroids without medical supervision.

Taking hormones unnecessarily can cause significant side effects and can interfere with the body’s natural testosterone production and fertility.

If testosterone replacement is medically appropriate, it should be prescribed and monitored appropriately.

Pay attention to the wider picture

Testosterone is important, but it should not become the explanation for every change in men’s health.

Persistent fatigue deserves investigation. So does a sustained loss of libido, reduced strength, recurrent erectile difficulties or unexplained changes in physical wellbeing.

Sometimes testosterone will be part of the explanation.

In other cases, the cause may involve cardiovascular health, sleep, stress, medication, mental health or another medical condition.

The most useful approach is therefore not trying to “maximise” testosterone at all costs. It is maintaining good general health and seeking appropriate assessment when something changes.

Testosterone plays an essential role throughout adult life, but healthy ageing depends on much more than a single hormone.
